Explicit Consent Text

Under Article 6 of Personal Data Protection Law No. 6698, health data is special category personal data. By confirming this text, you give your explicit consent to the processing described below.

Processing consented to

I give my explicit consent to the processing of information about my health that I share in appointment notes, contact requests and during sessions, for the purpose of providing the psychological counselling/psychotherapy service, by the data controller Eda Erkan Alapala (Safir İş Merkezi, Üçtutlar Mah. Dr. İlhan Gürel Cd. No: 42/2, 19000 Merkez / Çorum), for the purposes of delivering the service, scheduling appointments and creating the records that must be kept under professional ethics.

Transfer

Health data is not transferred to third parties; statutory obligations and the limits of confidentiality (e.g. risk to life) are reserved. For payment transactions, only payment details are transferred to the payment institution (iyzico); no health data is transferred.

Withdrawal of consent

You may withdraw your explicit consent at any time by notifying eda1erkan@gmail.com in writing. Withdrawal does not affect processing carried out up to that date; however, it may not be possible to continue the service without processing health data.
